SA Collaborative Projects

The Collaborative Projects are regional structures that facilitate service reform and improve service cooperation and coordination at a regional level.The projects engage with services that support frail older people and their carers.

The work completed by the projects will lead to improved quality of life outcomes for the target group, including independence and participation in their communities of choice through a partnership approach with all stakeholders.

The 2026 Regional Reform Forum Film Series 

This playlist contains segments of a short film which were recorded during March 2026, and screened at the Regional Reform Forum hosted on 11 May 2026 by Southern Fleurieu Positive Ageing Taskforce, Hills Positive Ageing Project, CO.AS.IT.SA, and Southern Services Reform Group, who provide Sector Support and Development funded by the Australian Government Department of Health and Aged Care.

The film explores perspectives from four different Commonwealth Home Support Programme (CHSP) providers - who deliver a range of service types (CALD, Social Support, Respite, Transport) - regarding the implementation of legislative requirements related to the new Aged Care Act, with a spotlight on Service Agreements, Associated Providers, Feedback and Complaints.

We extend our sincere appreciation to the organisations featured for generously sharing their experiences: Co.As.It SA, City of Victor Harbor, City of Onkaparinga, and Tailem Bend Community Centre.

Reflective Conversations: Advance Care Planning

These resources accompanied a Reflective Conversations webinar jointly provided by SSRG, Hills Positive Ageing Project and Grampians Sector Development Team. The webinar focused on how staff working in CHSP can introduce Advanced Care Planning conversations respectfully and in a way that supports client dignity, autonomy and cultural preferences. It explored CHSP responsibilities and boundaries related to Advance Care Planning, connection to the Strengthened Aged Care Quality Standards and communication, documentation and referral pathways.

Online learning related to the Serious Incident Report Scheme

These training resources were originally developed in 2024 as a partnership project between the Southern Services Reform Group (SSRG) and the City of Onkaparinga using Articulate 360. They have since been utilised by several Local Government Commonwealth Home Support Program (CHSP) providers, and have now been refreshed for 2026.

The package includes short, easy‑to‑access interactive online learning modules that can be accessed via the direct link. The link can be sent via SMS and the product right sizes to the e-device in use (phone/ tablet/PC) Individuals will need to provide an email address to register and access the modules.
